University Events with Community Participants

Are you having an event where community members are allowed to participate in?

If so, fill out the BYU–Hawaii Waiver Request for Community Participants form found below at least 30 days prior to the date of your event. Your request will be submitted to Risk Management for review and processing so form must be filled out completely in order to be processed. If there are any questions, you will be contacted.

You are required to provide a list of community participants to Event Services & Outreach with a copy of the signed waivers.

**If minors are participating, send a list of ALL department personnel assisting with event to Event Services & Outreach and have them complete the Protection of Minor training. Put the name of this event in the "Youth Affiliation" field.
